Paul Sartori Hospice at Home, a Pembrokeshire-based charity, were recently successful in obtaining a grant of £10,000 from The James Tudor Foundation. The Foundation, who support a wide and diverse range of health charities, awarded the grant towards the costs in supporting the Paul Sartori Registered Nurses. The team deliver end-of-life care to patients in the county, which amounts to over £200,000 per year.
The Nursing Team are experienced in end-of-life care and play an important part in the delivery of the holistic support offered by the charity. Paul Sartori offer a range of services to people living in the final stages of any life limiting illness, including home nursing care, complementary therapies, bereavement and counselling support, physiotherapy, future care planning and training. The home nursing team deliver thousands of hours of day and night respite per year to end of life patients in the community.
The Registered Nurses are crucial, playing an important part in supporting the patient and the family, which also enhances the provision of other local limited services. With their flexible and rapid approach, they support the patient’s wish to be cared for in their own home. They offer a patient-centred approach to care - assessing patients, referring to other internal services, work closely with other healthcare providers and are available as advisors. Nurses also assist with symptom management - administering medication when needed (to increase the comfort and wellbeing of the patient) and manage the 24/7 on-call facility (providing families with a point of contact in an emergency at all times of the day). The fast-track personal care package is coordinated by the team where they can support patients with care and equipment to ensure patients can return home from hospital.
“For family members, having a Paul Sartori Registered Nurse available and present within their home can offer great peace of mind.” said Laura Hugman, Clinical Team Manager at Paul Sartori Hospice at Home. “The nurses are experienced in their field ensuring dignity is maintained and quality of life is encouraged. With a Registered Nurse on site at night, patients may not need to wait as long for out-of-hours support or assistance,” added Laura.
“We are very grateful to The James Tudor Foundation in granting this donation towards the total costs. Our Nurses really make such a difference to local families and help to reduce anxiety at a very distressing time,” said Judith Williams, Grant Development Officer at Paul Sartori Hospice at Home.
The James Tudor Foundation’s core focus is the direct relief of human sickness. They have five funding programmes covering the full spectrum of relief of sickness. From medical research aimed at preventing and treating disease, to organisations that help individuals live with a particular condition, to palliative care charities that afford a caring and dignified end of life.
“The Foundation was very pleased to be able to contribute towards this much-needed service in Pembrokeshire. Providing the practical and emotional support people need at end of life is extremely important, and our Trustees were impressed by the highly effective way Paul Sartori delivers this support across such a rural county. We look forward to hearing more about the difference this service has continued to make for local families as the year progresses” said The James Tudor Foundation.
Paul Sartori Hospice at Home provide a wide range of other services to people living in the final stages of a life limiting illness, including: home nursing care, 24-hour support, standby service, complementary therapy, bereavement and counselling support, physiotherapy, future care planning and training.
These services enable people in the later stages of any life limiting illness to be cared for and to die at home with dignity, independence, pain free and surrounded by those they hold most dear if that is their wish.
